If you have the credentials:
1.Navigate to Find a Specific Member.
2.Enter patient Medicaid ID number or SSN, enter date of birth with no slashes (MMDDYYYY).
3.Click Search
To find out if patoent has active insurance

Call Carelon at 1-800-888-1965 for patient to opt out from other provider giving PRP and OP services. Patient must be present on the call.
