1.Navigate to Find a Specific Member.
2.Enter patient Medicaid ID number or SSN, enter date of birth with no slashes (MMDDYYYY).
3.Click Search
Patient Insurance Active? Yes

Call Carelon Customer Service.

MANDATORY STEP

Call Carelon Customer Service at 1-800-888-1965 for patient OPT OUT of existing PRP. Patient must be present on the call.

The following information will be requested NPI, Practice Name, Address of Practice.
